Fill in the following form, and the Make-A-Wish Foundation® will send you the full volunteer application packet in the mail. Please make sure to complete all information.
Please remember that:
Volunteers that participate in wishes must be 21 years of age.
Volunteers are required to attend an initial training session and continuing education sessions once every two years.
