How to Refer a Child

The referral process is easy:

We will accept a referral from medical professionals, parents/guardians and the potential wish child.

If you are anyone other than a parent/guardian or the potential wish child, you must first speak with the family and have their permission to contact the Make-A-Wish Foundation. We do not intrude on a family's privacy and want them to feel comfortable with the wish process.

We can accept referrals by phone, mail, fax or online.

You will need some basic information to make a referral including the child's name, medical condition and birthday, parents' names, address, phone number, and physician's name and phone number.
